Dale Leroy Middaugh, 78 of Polk passed away Friday April 30, 2021.  He was born May 31, 1942 in

Lodi the son of Melvin and Opal Dravenstott Middaugh.  He was a 1960 graduate of Polk High School

and worked for Guenther Plumbing & Heating.

He is survived by his wife, Cheryl Trapp Middaugh, to whom he was married June 9, 1972, two sons,

Aaron Middaugh and Andy (Veronica) Middaugh, all of Polk, a daughter, Amy (Randy) Sampson of

Ashland, two step-sons, Stephen Lynn Jefferies of Loudonville and Steven Carl (Donna) Smith of

Henderson, Nevada, 9 grandchildren, Paige, Kallie, Masilyn and Mia Sampson, Mayson and Sydnee

Middaugh, Gracie Lynn Jefferies, Brooke and Alex Smith.

He was preceded in death by his parents and a sister, Shirley Middaugh.

The family will have a celebration of life service at a later date.

Fickes Funeral Home in Jeromesville assisted the family with arrangements.
